In order to receive certain services or benefits, many agencies require a certified copy of a birth or death certificate. The Laclede County Health Department is linked to the Bureau of Vital Records for the State of Missouri, and can provide certified copies within just a few minutes of an application completion. Birth records are available from 1920 to present, and death records are available from 1980 to present. Our staff of registrars (Leslie, Angie, Janie & Charla) can assist you with records for Missouri birth and deaths. If you need a record for another state, we have telephone numbers and websites available to contact. The current state fees are: Birth Certificate - $15.00/per copy, Death Certificate - $13.00/1st copy, $10.00/each additional copy at original time of order. We have provided an application link for your use. This application can be used at the Laclede County Health Department (405 Harwood Avenue, Lebanon MO 65536). Mailed applications must be notarized. Please include payment and copy of identification.
